New Client Information and Forms

As a new therapy client, there are certain pieces of information that would be helpful for your therapist to know. Here is a list of common information requested by therapists during the initial intake process:

  1. Personal Information: Provide your full name, date of birth, contact information (phone number, email address), and home address.

  2. Emergency Contact: Share the name and contact information of a trusted person to reach in case of an emergency or crisis situation.

  3. Background Information: Briefly describe your current living situation, educational background, and occupation.

  4. Presenting Concerns: Describe the primary reason you are seeking therapy and the specific issues or challenges you would like to address.

  5. Medical History: Provide information about any physical health conditions, chronic illnesses, or disabilities that may be relevant to your therapy.

  6. Mental Health History: Share any previous mental health diagnoses, treatments, or therapy experiences you have had in the past. Mention if you are currently taking any psychiatric medications.

  7. Support System: Inform your therapist about your support system, including close friends, family members, or significant others who may play a role in your therapy or provide support outside of sessions.

  8. Allergies or Sensitivities: If you have any known allergies or sensitivities (e.g., to certain medications or environmental factors), make sure to mention them.

  9. Current Medications: List any medications you are currently taking, including prescriptions, over-the-counter medications, or supplements.

  10. Substance Use History: Provide information about your history of substance use or any concerns you may have regarding alcohol, drugs, or other addictive behaviors.
